What usually goes wrong
Following the protocol is critical, and the consultation is what makes it possible. Most service complaints trace back to a plan that was never explicitly confirmed, which turns a difference of expectation into a dispute about what was agreed.

When to sign someone off
A workable bar for this station: no critical items missed, a score above a line you set for Client Service specifically rather than one number for the whole building, and observed on more than one shift with at least one of them busy. Then date it, so it can expire.
That last part is the one people skip. A sign-off with no date cannot be re-checked, which means a year later you have a record that says somebody was ready once.

Because the record is for the next visit and for any reaction that appears afterwards. Documentation is the only thing that lets a different provider pick up the client correctly, and the only defence you have if something goes wrong later.
